Pensacola Pediatrics is excited to introduce our new check-in system! Check-in before your child’s appointment! We send you a text or email, you click the link, confirm your appointment, and check-in online prior to your visit.
We are rolling this out at our 6 locations over the next few months.
If you haven’t received a text or email at least 3 days prior to your appointment, please contact our office to ensure we have the most up-to-date contact information for you.
Please bring photo ID for you and the insurance card for the patient to every appointment.

Q: Do we need to use the system for every appointment?
A: Yes. After the initial registration, the next time will be faster as we save your answers.
Q: Do I need to download an app to do mobile check-in?
A: No app! Just click the link provided in the text message or email.
Q: What if I can’t finish before the appointment?
A: Talk to our receptionists. They will send you a new link. If you entered info, the system saved it and you start from where you left off.
Q: What if I have questions?
A: Call one of our offices if you have any questions.
Q: I lost my link. How do I get a new one?
A: Call your provider’s office to receive a new link.

Print, complete, and bring them to your upcoming appointment.
Patient Forms - Contains documents necessary for new patients. These forms are also used for updates of your child's demographic information each year.
Ages and Stages Questionnaires - Questionnaires to determine development of your child. These are necessary for routine well-child appointments.
Vaccine Resources - A list of resources to answer many common questions about vaccinations as well as our vaccine schedule.
Vanderbilt Forms - Forms that your child's doctor or school may request to have filled out.
